Certain medical services provided at Of Skin and Wellness may be eligible for insurance coverage. Coverage depends on your individual insurance plan, benefits, and the medical necessity of services rendered.

Insurance may apply to services such as:

• Primary care services
• Medical evaluations and follow-up visits
• Select laboratory testing
   

Insurance coverage varies by plan, and benefits are determined by your insurance carrier, not our office.

Patients are responsible for:

• Copayments
• Deductibles
• Coinsurance
• Any non-covered services
   

We recommend confirming benefits with your insurance provider prior to your visit.

Medical aesthetic and med spa services are not billed to insurance and are considered self-pay.

These services include, but are not limited to:

• Candela Matrix™ treatments
• Glacé™ treatments
• Microneedling and nanoneedling
• Chemical peels
• Cosmetic and wellness-focused services

Pricing for self-pay services is discussed during consultation to ensure recommendations align with your goals and individual needs..

Payment is due at the time services are rendered unless prior arrangements have been made.

Accepted forms of payment may include:

• Credit and debit cards
• Other payment methods as accepted by the practice
   

Our team is happy to review payment expectations with you before your appointment.

We kindly request at least 24 hours’ notice for appointment cancellations or rescheduling.

Appointments missed or canceled with less than 24 hours’ notice may be subject to a $50 missed appointment fee.

This policy allows us to respect provider time and accommodate other patients seeking care.
